This is only adsense placement i am using in my forum and this is working great

Are you sure its not against Google Adsense terms and conditions?
And I don't see these ad codes anywhere in my Adsense account.

Yes this is not against terms and condition.

That is simple 728x90 add. Since CTR is high then google is serving premium design
