EASY FORMS FOR vB 4.x here


Easily create forms with no HTML or PHP knowledge. Questions can be made compulsory and are checked before the form is submitted. Forms can be previewed. (You can create multiple forms)

Backwards compatible with vb v3.7.x and 3.6.8

Example Form | Help File | Version History

Question Types:
  • Single line text
  • Multi-line text
  • Multi-select checkboxes
  • Single-select radio buttons
  • Single Select Dropdown
  • Yes/No
  • Multi-line text field using vbeditor (max one per form - limited by vB)
  • Human verification question
  • Custom Question (can query database or do whatever you want)
  • Date (month/ day/ year fields)
Form can Submit to:
  • new thread in forum
  • new post in thread
  • new pm to user
  • email address
  • save to database
Installation:More Features:
  • Form list categories (optional)
  • Preview form (can disable for individual forms)

  • Prefix support - you can choose which prefix you want thread to have
  • Different thread/pm title to form title with variables {username} {userid} and {q_1} (replacing 1 with appropriate question number)

  • Usergroup permissions per form

Dynamic respsonsive dropdown select boxes.

No one answered my question on dynamic java select boxes so I spent a while messing around with custom questions until I got it working. Figured I'd share my findings in case anyone else has a similar issue.

This is probably not the best way to go about doing this but I had little choice so bare with me.

My problem, I needed select boxes that would change depending on their parent select box. IE Dynamic java select boxes.
I used Mirko Elviro's Multiple Dynamic Combo Boxes code available here.

I put the javascript in the form_view template, so yes it's added in every form regardless of whether or not those questions are used on that form. I couldn't find a way around it.

Block Disabled:      (Update License Status)  
Suspended or Unlicensed Members Cannot View Code.

And for each seperate custom question I did the following.

are there any potential security issues with this hack considering that it might allow hackers to plant malicious code into the server.....

can anyone give some basic analysis? tks.

hello there i need to know how to tweak this thing out

when you do the check boxs in more then one row there not all even (see pic)

and the other one is then you make a section when they post there input it post the section then (Single Line Text Field) is right next to it and not down one line (see other pic)

It is possible to replace the text in Dropdown Select by image?
For example, if the user selects (in the form) a number "1", how can I get (in the topic) "1" replace with image 1.jpg?

Am i doing this wrong. I uploaded the Clientscript files. Imported the xml file.

When i went to the admincp options it gave me the list of usergroups (ie admin mod etc...) not the usergroups ID. I couldn't add or delete usergroups.

I also had no permissions to access the forms page and couldn't create new forms either?

Encoding problems

Hi everyone.First of all thanks for such a great mode.
I have installed the mode and have some issues with encoding. I can create forms with russian characters, they are not displayed correctly. How can i change the encodding to UTF-8?

Second question is about attachments, is it possible to send attached files in forms to emails?

You need to highlight usergroup for grant permission. Use CTRL+CLICK for highlight more usergroups.

I have this modification installed, I rember before there was an incredibly detailed and effective list of how to go about doing certain things with this modification. However that link is now down. Does anyone know where I could possibly find the list still, or if someone could post perhaps how to add prefix's to the form. Although Id much rather get my hands on that list again. Thanks in advance if anyone is willing to help me out.

Would it be possible to make option so you can receive emails in the same format as the BB Codes for the output. Right now I still get that generic TXT emails instead ones with the format of the output. Thanks

I have this installed and when I try and create a form and submit it its telling me that I have too many characters? How can I increase the character limits?
